Instructions: 

1. Print the form below and fill it out. Blank forms are also available at our offices in McLouth and Topeka.
2. You can bring it to the office between September 23 and September 26 to pick up your wristbands ahead of the event for the fastest entry into the venue. 
3. Please bring it to Gary's Farm Fest on the day of the event, stop by the registration table and get your wristbands.
